The Lancet Commission on a Citizen-Centred Health System for India, published in January 2026, analyses the progress towards achieving universal health coverage for the people of India and proposes six major reforms to realize this goal. Building off a large body of primary and secondary research, the Commission shifts the conventional narrative of the barriers to realising UHC in India: these are no longer driven by the lack of political will, underfunding, inadequate human resources and physical infrastructure, or low health-seeking behaviours; instead, uneven quality of care, inefficiencies in spending, fragmented delivery, inadequate design and implementation of financial protection programmes, and poor governance emerge as key challenges. The clarion call is for a publicly-financed and publicly-provided integrated citizen-centred healthcare delivery system as the primary vehicle for UHC, while shaping the private sector to leverage its strengths.
